OK well something is definitely not right here as you say but it seems deeper than a driver issue to me, especially considering you have formatted to no avial. Do you have another system to try the card in? I had a similar issue with performance many moons ago on a 9600pro plus certain drivers just wouldn't work but the card worked fine in other systems and other cards worked fine in mine.
Could the PCIe have somehow changed to a 1x, or something?
May be worth having a review of the BOIS settings?
Oh, what about other games/benchies, if they're OK it may be a game specific issue, have you checked the game's sites, to see it others are having issue and if there's a fix there?
This problem usually occurs which you corrupt your driver cache somehow (Usually using sweepers when not in VGA mode etc etc.) but thankfully its a fairly easy fix....
Hit start and search for Gpedit.msc and select it. Now select in computer configuration: Administration templates / system / device installation restrictions / prevent installation of devices not described by other policy and enable it.
This will BLOCK windows from installing driver from its cache or anywhere else.
Now got to device manager an uninstall your card from there whilst selecting delete driver software as you go. Now go to add or remove programs and remove everything else AMD / NV.
Reboot and rejoice as Windows boots into VGA mode with a notification saying driver install blocked..... As you are in true VGA mode feel free to sweep, delete and do what you like as no drivers are installed to be damaged. (Not that I believe in Sweepers.) Do not to reboot after sweeping and deleting so go into Gpedit and reverse what you did then (Do not reboot as the corrupt driver will be installed again.) re-install your drivers and all should be gravy again.

Nice how you can Google stuff kitch. Not everyone uses Professional or Ultimate and still have those issues.
Been using that for years, and yes Google does help when you have problems you are struggling with.
